If you are a driver with a lost driving license or a stolen driving license, you may request a duplicate in one of four ways: in person, online, by mail or by phone. See below which option is most convenient for you.
Report Stolen Drivers License
Prior to visiting your nearby MVD office, make sure to stop by a local police station to file a report on your lost driving license or stolen driving license. By notifying the proper authorities of your incident, you will reduce your chances of identity theft.
Replace a Drivers License in Person
A drivers license replacement at a local MVD office will require you to provide two proofs of identity. One of your documents must be a primary item, with a photograph of yourself and your date of birth. If you cannot submit a primary item, then you will need to hand in three documents from a secondary list instead.
Some acceptable documents include the following:
- Arizona ID card
- United States passport
- United States passport card
- United States birth certificate
- United States Certificate of Naturalization
- United States Certificate of Citizenship
- Foreign passport with valid immigration document or stamp
- Tribal Certificate of Indian Blood
- Tribal or Bureau of Indian Affairs Affidavit of Birth
- United States Military Card
- Affidavit of Identification
- Arizona Certificate of Birth
- Bank card or credit card
- Employee ID card with photo
- U.S. Department of Veterans Affairs Card
- Social Security card
- Marriage Certificate
- W-2 Form
- Concealed Weapons Permit
- Legal Guardian Certificate
- Medical insurance ID card
Replace a Drivers License Online
You can quickly request an Arizona duplicate drivers license by visiting the ADOT Duplicate Driver License Online Services page. You will need to have a credit card or debit card available to submit online payment for the replacement fee of $12. Only Visa, MasterCard, American Express or Discover credit cards are accepted.
Replace a Drivers License by Mail
Your mail-in duplicate request can be completed by sending a written letter with your personal information, along with proper payment, to the address below:

Make sure to include your AZ drivers license number when writing your check payable to Motor Vehicle Division. You can expect to receive your drivers license replacement within four weeks once your request has been processed.
Replace a Drivers License by Phone
To get a drivers license copy by phone, call the Arizona Motor Vehicle Division at 602-255-0072 (if you are in the Phoenix area) or at 520-629-9808 (if you are in the Tucson area). State drivers in other parts of AZ should call 1-800-251-5866. Hours of operation for the customer service line is Monday to Friday, from 8 a.m. to 5 p.m. You will be required to have payment readily available for the $12 fee. Only credit cards are accepted by phone.
